Key Responsibilities
  • Implement behavioral modification techniques, assertiveness training, and supportive therapy to assist clients in achieving their therapy goals.
  • Conduct thorough assessments of patients and their families by effectively gathering data through interviews, collaboration with the healthcare team, and reviewing patient charts.
  • Develop and periodically revise personalized treatment plans based on comprehensive psychosocial assessments that take into account the patient's individual concerns, strengths, cultural values, and needs.
  • Create and facilitate educational and therapeutic group sessions, documenting patient progress and goals in therapeutic activities to monitor their development.
  • Serve as a vital link between patients and community resources, ensuring that patients receive comprehensive care and support beyond clinical settings.
  • Maintain accurate and timely documentation of services provided, including treatment plans, client interactions, and progress notes, complying with health regulations and organizational standards.
Patient Population / Client Focus

Your primary focus will be on patients from diverse backgrounds within the community who present various medical, social, and emotional challenges. Working with individuals and families grappling with mental health issues, trauma, and life transitions, you will apply a culturally competent lens to your practice, ensuring respectful and effective care. The role encompasses advocating for clients' needs and fostering resilience within families.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Active licensure as a Clinical Social Worker (LCSW) in the state of California.
  • Exceptional written and verbal communication skills to effectively interact with clients, families, and team members.
  • Strong ability to establish and maintain appropriate boundaries with clients, promoting a safe therapeutic environment.
  • Detail-oriented with proficiency in generating documentation in a timely manner, including treatment notes and progress reports.
Preferred Experience
  • Experience working in a community health setting or similar environment is highly desirable.
  • Familiarity with telehealth platforms and remote patient engagement methods.
  • Demonstrated ability to develop and implement group therapy programs.
  • Experience in collaborating with multidisciplinary teams to enhance patient care.
Compensation and Benefits

We offer a competitive salary ranging from $120,000 to $130,000 annually, based on caseload and experience.
