Key Responsibilities
  • Engage directly with clients through telehealth consultations, conducting assessments and providing therapy tailored to individual needs.
  • Participate actively as a collaborative team member, sharing insights and strategies to enhance service delivery and improve client outcomes.
  • Work effectively with a diverse client base, demonstrating respect and understanding for varying beliefs, traditions, and interpersonal styles.
  • Develop and implement integrated care plans, ensuring continuity of care by facilitating communication among clients, their families, and healthcare providers.
  • Conduct brief, evidence-based screenings and arrange for more comprehensive evaluations when needed.
  • Incorporate crisis intervention techniques to support clients in acute distress, ensuring safety and appropriate responses to crisis situations.
  • Maintain accurate documentation of client interactions, treatment plans, and progress reports, adhering to professional and organizational standards.
  • Monitor clinical outcomes, adjusting treatment plans as necessary to ensure that client progress is optimized.
  • Provide on-call support during operational hours, allowing for immediate assistance to clients who may require urgent care.
Patient Population / Client Focus

This role specifically focuses on clients facing significant challenges related to major mental illness and severe emotional disorders. The clientele served often come from underprivileged backgrounds, requiring a tailored approach to treatment that encompasses psychological, social, and community-based support. You will engage with clients in a hospice center setting, underscoring the importance of compassionate care during times of vulnerability.

Qualifications
  • A valid and current Licensed Clinical Social Worker (LCSW) license.
  • Educational background with a Bachelor’s degree in Social Work or a related field.
  • A minimum of two years of relevant experience in a clinical setting.
  • Strong understanding of mental health diagnoses, treatment modalities, and evidence-based practices.
  • Ability to work independently as well as part of a collaborative team.
Preferred Experience
  • Experience working with hospice or palliative care populations is a plus.
  • Proficiency in using electronic health record (EHR) systems and telehealth platforms.
  • Familiarity with community resources to support client needs effectively.
  • Background in crisis intervention and management techniques.
Compensation and Benefits

The salary for this position ranges from $70,000 to $85,000 annually, commensurate with experience and qualifications.
