Position Overview

Bright Path Social Work is seeking a skilled and compassionate Licensed Master Social Worker (LMSW) to join our dedicated team. Work Environment

As a remote position, this role allows for flexible scheduling, primarily Monday through Friday from 8:00 AM to 4:30 PM. However, candidates should be prepared to adapt their availability for occasional evenings or weekends to meet client needs and ensure consistent access to care. Being on-call to address urgent client situations may also be a requirement. Our team utilizes telehealth methods alongside traditional in-person interactions, ensuring comprehensive and responsive service delivery.

Key Responsibilities

• Conduct in-depth assessments to evaluate the psychosocial needs of seniors, focusing on emotional, mental, and social factors that affect their lives.
• Develop individualized care plans in collaboration with clients and their families that address specific goals, preferences, and long-term care needs.
• Provide ongoing counseling and emotional support to clients and families, helping navigate transitions such as moving into assisted living or dealing with bereavement.
• Advocate for clients to ensure they have access to essential community resources and services appropriate to their circumstances.
• Coordinate with an interdisciplinary team, including healthcare providers and caregivers, to enhance the care and support provided to clients.
• Perform regular home visits to monitor clients’ living conditions, assess well-being, and proactively address any safety concerns.
• Educate clients and families about local resources, support programs, and community engagement opportunities.
• Maintain comprehensive and accurate documentation of client interactions, assessments, and progress notes in accordance with compliance guidelines.

Patient Population / Client Focus

This role primarily focuses on elderly clients facing various life challenges, including physical health deterioration, mental health concerns, and social isolation. By understanding the complexities of aging, you will help clients maintain autonomy while ensuring they receive necessary support and resources. Your expertise will directly contribute to enhancing their everyday experiences and overall well-being.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current, valid licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.
• A minimum of two years of experience in social work, particularly in geriatric or community health settings.
• Solid understanding of social work principles and best practices, especially relating to aging issues.
• Exceptional skills in assessment, problem-solving, and empathetic communication.
• Ability to forge strong relationships with clients and their families through compassion and understanding.
• Proficiency in utilizing electronic health record (EHR) systems and other relevant technology.

Preferred Experience

• Experience working in a community health clinic or similar environment, connecting seniors with resources and support.
• Familiarity with telehealth practices and digital communication tools.
• Knowledge of local community resources available for seniors and families in distress.

Compensation and Benefits

The salary for this position ranges from $60,000 to $100,000 per year, commensurate with experience and qualifications.
