Key Responsibilities
  • Conduct comprehensive psychosocial assessments through in-depth interviews, detailed observations, and thorough reviews of medical records to identify and evaluate emotional, social, and environmental factors impacting patients’ recovery processes.
  • Deliver personalized counseling and advocacy to patients and their families, facilitate discussions about adjustment challenges, and provide emotional support to promote mental well-being.
  • Collaborate effectively with a diverse rehabilitation team, including therapists and medical professionals, to develop holistic and individualized treatment plans tailored to the psychosocial needs of each patient.
  • Assist patients in connecting with essential community resources such as vocational rehabilitation programs, support groups, and housing assistance, helping them navigate post-rehabilitation transitions seamlessly.
  • Educate patients and their families about available community resources and post-rehabilitation care, ensuring a clear understanding of next steps in the recovery journey.
  • Engage in discharge planning to facilitate smooth transitions for patients returning home or shifting to alternative care environments, contributing to successful outcomes and continued independence.
Patient Population / Client Focus

The role focuses on a diverse patient population, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Our approach aims to address not only the physical aspects of rehabilitation but also the emotional and social dimensions crucial for holistic patient recovery.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.
  • A strong foundation in adjustment issues, grief, coping mechanisms, and their application in a clinical setting.
  • Excellent communication and interpersonal skills to foster trusting relationships with patients and families.
Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 75,000 to $100,000 annually, alongside opportunities for performance bonuses.
