Key Responsibilities
  • Conduct thorough assessments of clients' psychosocial needs to formulate individualized treatment plans that address emotional, spiritual, and practical aspects.
  • Deliver evidence-based therapy sessions in various formats, including individual and family counseling, utilizing methods like cognitive-behavioral therapy and mindfulness techniques.
  • Communicate and collaborate with family members and caregiving teams to create a supportive network for clients, ensuring everyone is aligned in the care process.
  • Provide crisis intervention when necessary, employing de-escalation techniques to manage high-stress situations effectively.
  • Document clinical progress in accordance with HIPAA regulations and organizational policies, ensuring consistent updates in electronic health records (EHR).
Patient Population / Client Focus

In this role, you will primarily work with individuals facing terminal or life-limiting diagnoses, along with their families. These clients often require sensitive emotional support as they navigate complex feelings associated with their health conditions. Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • An active Licensed Clinical Social Worker (LCSW) license issued by the state of California.
  • A minimum of two years of clinical experience, preferably within hospice or palliative care settings.
  • Strong organizational, documentation, and case management skills, with proficiency in handling sensitive information.
Preferred Experience
  • Experience using electronic health record systems to document client interactions and treatment outcomes.
  • Familiarity with hospice regulations and palliative care best practices.
  • Skills or experience in restorative practices and shared decision-making processes.
