Work Environment

This position offers the flexibility of a remote work setting, allowing you to manage your schedule while meeting the needs of your clients. While the role is primarily home-based, some evening and weekend hours will be required to ensure timely support for our patients. This adaptable work arrangement enables our team members to balance professional commitments with personal responsibilities effectively.

Key Responsibilities
  • Utilize your clinical training and experience to conduct thorough psychosocial assessments, providing independent diagnoses for a diverse array of patient cases.
  • Collaborate with patients and their families to co-create individualized treatment plans aimed at achieving specific mental health goals.
  • Implement evidence-based therapeutic interventions, including but not limited to solution-focused therapy, motivational interviewing, and problem-solving treatment approaches.
  • Engage in regular coordination with other healthcare professionals, including psychologists and primary care providers, to ensure an integrated approach to patient care.
  • Proactively track patient progress through systematic monitoring and adjustment of treatment plans to improve outcomes.
  • Assist in the management of psychotropic medications as prescribed, focusing on adherence, potential side effects, and overall treatment efficacy.
Patient Population / Client Focus

Your work will primarily focus on patients within a hospice context, catering to individuals facing significant health challenges, including terminal illnesses. You will provide essential emotional and psychological support to patients and their families during vulnerable times, ensuring that their wishes and needs are met with dignity and respect.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current, valid, and unrestricted license as a Licensed Clinical Social Worker (LCSW) in the state of Florida.
  • A minimum of one year of experience in a clinical setting as an independently licensed therapist, demonstrating a solid foundation in therapeutic practices.
Preferred Experience

While a minimum of one year of experience is required, we highly value applicants with additional experience in hospice or palliative care settings. Familiarity with grief counseling and end-of-life issues will enhance your capacity to provide compassionate support to patients and their families.

Compensation and Benefits

The salary for this role ranges from $80,000 to $100,000 annually, supplemented by an annual incentive bonus based on performance. Additionally, we offer a comprehensive benefits package that includes:
