Work Environment

Key Responsibilities
  • Utilize your education, training, and experience to conduct thorough psychosocial assessments, formulating independent diagnoses in accordance with best practices.
  • Work collaboratively with patients to design comprehensive, patient-centered treatment plans that address their unique needs and goals.
  • Provide evidence-based behavioral interventions, which may include techniques such as solution-focused therapy, motivational interviewing, and problem-solving treatment.
  • Coordinate effectively with other healthcare professionals, including psychologists and primary care providers, to deliver integrated care that fosters better health outcomes.
  • Track treatment responses systematically, monitoring progress toward established goals through both in-person and virtual check-ins.
  • Support patients in managing psychotropic medications as prescribed, focusing on treatment adherence and understanding side effects to enhance overall efficacy.
Patient Population / Client Focus

You will work primarily with patients in hospice care, navigating the emotional challenges associated with end-of-life issues. This role requires a sensitive approach to working with individuals facing serious illnesses, as well as their families, ensuring a supportive and compassionate environment. Your contribution will directly influence patient well-being, providing essential emotional and psychological support during a critical phase of their lives.

Qualifications
  • A Master’s degree in Social Work from an accredited program.
  • A current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.
  • A minimum of 1 year of experience as an independently licensed therapist, demonstrating the ability to handle complex cases with care and compassion.
Preferred Experience
  • Experience in hospice or palliative care settings, understanding the unique needs surrounding end-of-life care.
  • Familiarity with evidence-based practices specific to mental health treatment and an ability to implement them effectively.
  • Previous collaboration within interdisciplinary teams and understanding the importance of a coordinated care approach.
Compensation and Benefits
