Position Overview

We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team at Adaptable Human Solutions Clinic, a leading community health clinic located in Mesa, Arizona. This full-time, remote position is a unique opportunity for a passionate social work professional to provide essential mental health support and empower individuals, particularly seniors, in navigating various life challenges. Work Environment

This role is primarily remote, allowing you the flexibility to manage your work-life balance while still actively engaging clients in a community health setting. Our traditional work hours are from Monday to Friday, 8:00 AM to 4:30 PM, though occasional evening and weekend shifts may be necessary based on client demands. Flexibility is key, as some on-call availability may be required to address urgent client needs or provide support beyond regular hours.

Key Responsibilities
  • Conduct comprehensive assessments of seniors’ psychosocial requirements, considering emotional and social factors.
  • Collaborate closely with clients and their families to devise customized care plans that align with their individual goals and preferences.
  • Provide professional counseling and emotional support, facilitating clients in navigating life transitions, grief, and coping strategies.
  • Advocate for clients’ rights, ensuring they have access to necessary community resources and services tailored to their needs.
  • Engage with interdisciplinary teams that include nurses, caregivers, and healthcare professionals to enhance integrated care delivery.
  • Conduct regular check-ins with clients to monitor their well-being, reassess living conditions, and address safety concerns as they arise.
  • Facilitate effective communication among family members, helping to resolve conflicts that may arise in the client’s care.
  • Educate clients and their families on the various community resources available to them, including support groups that foster social connection and improve overall quality of life.
  • Maintain meticulous documentation of client assessments, care plans, progress notes, and interactions to ensure continuity of care and compliance with regulatory standards.
  • Stay updated on current social work practices and policies, ensuring adherence to ethical standards in all interactions.
Patient Population / Client Focus

Your primary client focus will be seniors, a demographic that often faces unique emotional and social challenges. Understanding and addressing the psychological impact of aging, health concerns, family dynamics, and community connections will be key to your success in this role. By coordinating with family members and other care providers, you will foster an environment that supports emotional resilience and autonomy for each client.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is a requirement.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A minimum of two years of social work experience is essential, preferably in a community health or geriatric setting.
  • Demonstrated expertise in social work principles, with a strong foundation in aging-related issues.
  • Exceptional assessment, counseling, and problem-solving abilities.
  • Strong empathy skills, allowing for rapport-building with seniors and their families.
  • Excellent communication skills that facilitate successful collaboration with interdisciplinary teams.
  • Proficiency in using electronic health record (EHR) systems and standard computer applications.
Preferred Experience
  • Experience working with older adults, especially in community centers or healthcare settings.

Compensation and Benefits
