Work Environment

Key Responsibilities
  • Conduct ongoing assessments to establish tailored treatment plans for patients, ensuring their mental health needs are met effectively.
  • Facilitate communication among supervisors, support staff, family members, and other stakeholders to track individual progress and adapt treatment as necessary.
  • Provide supervisors with a proactive schedule of appointments, home visits, and observational tasks for consistent monitoring of patient advancements.
  • Coordinate and execute home visits to gather essential data through in-home observations, chart reviews, and interviews with patients and their families.
  • Ensure that treatment program plans are implemented thoroughly and with the highest fidelity.
  • Participate in interdisciplinary team meetings to contribute insights and collaborate with other healthcare professionals in the clinic.
  • Document and maintain comprehensive progress notes on a monthly and quarterly basis, ensuring accurate tracking of patient care.
  • Organize and facilitate training sessions for staff members to ensure competency in implementing program plans and utilizing documentation protocols.
  • Conduct follow-up interviews and home visits to provide ongoing support tailored to individual patient needs.
  • Act as a liaison between the clinic and the broader mental health community, advocating for patient-centered development goals.
  • Contribute to the creation of new training materials and procedures to continually enhance the quality of programs for patients.
Patient Population / Client Focus

In this role, you will work with a varied demographic, including both youth and adults, tackling a range of mental health challenges such as anxiety, depression, trauma, and substance misuse. Your expertise will support patients in navigating their life circumstances and achieving their personal wellness goals.

Qualifications
  • A Master’s degree or Doctoral degree in Counseling, Psychology, or a related mental health field is required.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Colorado.
  • At least two years of counseling experience, preferably in a clinical context, is required to understand the complexities of patient care.
  • Preferred specializations include family therapy, couples therapy, substance misuse, or trauma-focused services.
  • A strong understanding of legal and ethical standards in mental health practice.
  • Exceptional interpersonal and communication skills, capable of building strong rapport with diverse clientele.
  • Demonstrated ability to collaborate effectively within a multi-disciplinary healthcare team.
  • Proficiency in electronic medical record (EMR) software and related technologies for documentation and patient management.
Preferred Experience
  • Experience in community mental health settings or similar environments where integrative care approaches are utilized.
  • Familiarity with evidence-based practices for treating various mental health conditions.
  • A background in leading staff training initiatives or workshops in counseling techniques or ethical practice.
