Zenith Mind Counseling Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in Naperville, Illinois. This position is primarily remote, allowing for flexibility in your work schedule while collaborating with colleagues across the organization. You'll have the opportunity to contribute to our hospice center, carefully designed to offer compassionate care and support to patients nearing the end of life. • Act as a liaison between patients, families, and outpatient providers to ensure cohesive care plans.
• Engage with insurance companies for pre-certification and utilization management, ensuring proper coverage and access to necessary resources.
• Participate actively in discharge and aftercare planning, coordinating follow-up care and services.
• Provide therapeutic meal support, guiding clients in understanding and modeling a healthy relationship with food.
• Deliver empathetic and culturally sensitive care, recognizing and affirming the unique circumstances of each client by considering their intersecting identities.

Your primary focus will be on older adults in a hospice care setting, providing critical support during one of the most sensitive times in their lives. You will work closely with patients dealing with terminal diagnoses, offering emotional and psychological support while ensuring their needs are prioritized alongside the families who also require guidance and assistance.

• A Master's degree in Social Work from an accredited institution is required.
• Must hold a valid LCSW license in the state of Illinois.
• Demonstrated experience working with older adults and an understanding of their specific psychological and emotional needs.
• Strong clinical skills, including diagnostic and assessment capabilities.
• Exceptional communication and interpersonal skills, with a focus on building rapport and trust with clients and colleagues.

While not mandatory, candidates with experience in hospice care, end-of-life counseling, or similar settings will be highly regarded. Familiarity with case management software and electronic health records will also be beneficial.
