Job description

**New Hire Bonus: $1000

Looking to make a difference for youth and their families? Do you want to have a lasting impact on their future?
Hope For Youth is seeking a dedicated Therapist to join our Youth Assertive Community Treatment (ACT) Program.
Youth ACT is a program licensed by the New York State Office of Mental Health and designed to address the significant needs of children ages 10 to 21 who are at risk of entering-or returning home from-high-intensity services such as inpatient or residential treatment. Using a multi-disciplinary team approach, Youth ACT provides intensive, community-based support to help children with serious psychiatric needs remain safely in their homes whenever possible.
Key Responsibilities:
  1. Function within the policies and procedures as outlined in program practice guide and employee manual
  2. Adhere to state regulations and applicable code of conduct
  3. Work within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomes
  4. Conduct person-centered/individualized Family, Individual, and Group Therapy in the home, community and office spaces
  5. Ensure that service delivery is in a trauma informed, culturally and linguistically competent manner
  6. Attend trainings and professional development as assigned
  7. Liaise with DSS, schools and other child and youth collateral contacts
  8. Attend daily team meetings and family meetings as assigned
  9. Conduct comprehensive clinical assessments to diagnose and/or determine needs
  10. Develop a treatment plan with the child/family to restore functioning and/or ameliorate behavioral health symptom.
  11. Apply agency-identified Evidence Based Interventions best suited to meet the client's needs.
  12. Provide transportation for client to their service location as necessary.
  13. Provide crisis management and be on a rotation to cover crisis outside normal business hours.
  14. Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations

How You'll Spend Your Time:
60% Direct client therapeutic contact including transportation
20% Case documentation, reports, case related meetings
20% Staff/team meetings/supervision/training
To qualify for this job, you need to be or have a minimum:
  1. LMSW/LCSW/LMHC/LMFT, limited permits
  2. 1 years' experience working with children/adolescents and families preferred
  3. Computer proficiency, including Microsoft Office Suite, and experience working with EHRs.
  4. Valid NYS driver's license in good standing
  5. Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, and ongoing daily access to own vehicle
