JOB DESCRIPTION
Position Title: Care Coordinator III, Young Adult Court
Employment Classification: Full Time/Non-Exempt
Shift: Monday - Friday 9:00 AM - 5:30 PM with some flexibility for evening workshops.
Reports to: Young Adult Court Supervisor
Department: Programs
Program Summary: The Young Adult Court Program (YAC) works in collaboration with University of California Irvine, Orangewood Foundation, the District Attorney's Office, Defense Attorney's Office, and The Orange County Superior Court. The YAC program provides engagement, assessment, care coordination and resource coordination support to Transitional Age Youth (ages 18-25) as part of a collaborative research study to reduce recidivism.
Position Summary: We are seeking a dedicated and compassionate Young Adult Court Care Coordinator to join our team. The ideal candidate will utilize a trauma-informed, person-centered approach and employ motivational interviewing techniques to work alongside participants in the Young Adult Court program. This program, which spans 18-24 months, is designed to support young adults through a highly collaborative approach that includes Four stages of support: Engagement, Accountability, Stability & Sustainability, and Attainment.

Requirements
Major Areas of Responsibility:
  • Work closely with participants to create individualized goals and provide ongoing support to meet their basic needs and build essential life skills, including crisis intervention.
  • Case management aims to reduce barriers and challenges for youth by meeting them in the community, at the office, in custody jail or in their homes.
  • Facilitate and participate in client orientations, workshops, and prosocial activities.
  • Facilitate youth support group meetings.
  • Utilize trauma-informed practices to foster a supportive and empowering environment for participants.
  • Employ motivational interviewing techniques to effectively engage and motivate participants throughout their journey in the program.
  • Collaborate with a multidisciplinary team to ensure comprehensive and cohesive support to participants.
  • Monitor and document participant progress, providing regular updates and reports to the team, including the court.
  • Assist participants in accessing and navigating community resources and services to support their goals and needs.
  • Additional duties as assigned by program leadership.
Knowledge, Skill, and Experience Requirements:
  • Bachelor's degree in Criminal Justice, Human Services, Sociology, Social Work, Education, Psychology, or a related field required.
  • 3+ years of experience in a mental health, social services, collaborative court, or community services organization providing direct services to the target population, OR 1 year of direct service experience combined with significant lived experience relevant to the population served.
  • Experience working with transitional-age youth, particularly those involved in the criminal justice system.
  • Strong understanding of trauma-informed care, harm reduction, motivational interviewing, coaching, and mentoring practices.
  • Excellent verbal and written communication, interpersonal, and relationship-building skills.
  • Ability to work collaboratively within a multidisciplinary team and coordinate services effectively.
  • Demonstrated ability to work sensitively and effectively with youth impacted by trauma.
  • Strong organizational, documentation, and data entry skills, including proficiency with electronic database systems and standard computer applications.
  • Ability to identify opportunities and challenges, think critically and strategically, and make sound and timely decisions under pressure.
  • Ability to maintain confidentiality, accept and implement feedback, and demonstrate professionalism and accountability.
  • Commitment to fostering an inclusive, equitable, and supportive environment for all participants.
  • Possess a valid California driver's license and proof of automobile insurance. Position requires DMV clearance.
  • Pre-employment screenings include criminal background check, FBI/DOJ fingerprinting, TB test, and drug screen. Previous employment will be verified.

Working Environment/Physical Requirements:
  • Available to work evenings and weekends as needed
  • Position may require use of personal vehicle. Physical requirements include standing, sitting, typing, bending and lifting up to approximately 25 lbs. (i.e. tables, chairs, food, water, etc.).
  • Modification to work schedule may be made based on needs of the Foundation.
