Job description

The WRT BN works as part of a community-based Wellness Team consisting of a registered nurse (PN) and mental health clinician (BN). The BN brings to the team clinical expertise in the assessment, treatment, and management of co-occurring behavior and physical health concerns. Within this team, the BN also has specific knowledge, skills, and abilities in the arena of primary care behavioral health integration. The BN will work one-on-one with their caseload to ensure overall health management, limiting inpatient hospital stays and connecting individuals to appropriate resources for increased quality of life.
Reports to:
The WRT BN reports to the WRT Administrative Navigator and communicates with all members of WRT Team and the Executive Director.
Responsibilities and Duties:
  • Provides outreach and engagement to build rapport and explain the benefits of integrated care and how the team can assist in improving a memberโ€™s overall health.
  • Complete member assessment and Smart Screener
  • Identify and address needs of clients
  • Reviews MH evaluations and collaborate with MH providers
  • Develop strategies with clients on self-management, diet/nutrition, exercise, use of preventative care, etc.
  • Provide psychoeducation to clients on managing medical conditions
  • Attends and advocates at client appointments as needed.
  • Participates in WRT Case Reviews
  • Communicates with collaborative partners and clientโ€™s support system
  • Maintains clinical caseload and charts according to QA and program polices.
  • Maintains billable expectations per week
  • Attends clinical supervision with direct supervisor.
  • Completes clinical notes in a timely manner
  • Follows all HIPPA and agency policies regarding client confidentiality and privacy.
Job Requirements:
  • This position requires a masterโ€™s degree in behavioral health
  • Knowledge of community resources and integrated health
  • Current valid Pennsylvania driverโ€™s license
  • Current Pa Child Abuse Clearance
  • Criminal background clearance
