Job description

We are seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Summit County. The Case Manager will assist with accessing community based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have BA in related field, one year related experience and knowledge of community resources.

Job Duties of Case Manager includes, but not limited, to:

  • Provide short-term case management and wraparound services to adults and families
  • Transition clients and families into ongoing care
  • Work collaboratively with treatment teams internally and externally to ensure safe transitions

Qualifications of Case Managers Mobile Response:

  • Bachelor's degree in a social service-related field
  • Willing to get NPI and Medicaid Billing number (we'll help!)
  • Maintain excellent driving record
