Position Overview
We are seeking a skilled and motivated Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients achieve their health goals.
Responsibilities

  • Perform in-home patient assessments and skilled nursing visits
  • Develop and update individualized care plans
  • Perform wound care, disease management, medication administration, and patient education
  • Communicate effectively with physicians, case managers, and interdisciplinary teams
  • Complete accurate, timely documentation in accordance with regulatory requirements
  • Participate in on-call rotation as needed
  • Build trusting relationships with patients and families
Qualifications
  • Current RN license in the state of Texas
  • 1+ year of nursing experience (Home Health preferred but not required)
  • Strong clinical assessment and critical-thinking skills
  • Reliable transportation, valid driver's license, and proof of auto insurance
  • Comfortable using EMR/mobile charting
  • Compassionate, professional, and patient-focused
