We are seeking an experienced, energetic, and compassionate Registered Nurse (RN) to join our leadership team as a Clinical Services Manager, RN – Home Health.

This is an outstanding opportunity for a home health leader who enjoys mentoring clinicians, overseeing patient care operations, and making a meaningful impact on both patients and staff.

Position Summary

The Clinical Services Manager is responsible for the day-to-day management and supervision of clinical operations and multidisciplinary home health teams. This position ensures timely, high-quality patient care, supports clinician development, promotes regulatory compliance, and assists in driving excellent patient outcomes.

Essential Responsibilities

  • Provide daily supervision and support to Registered Nurses, Licensed Practical Nurses, Home Health Aides, Physical, Occupational and Speech Therapists, and Medical Social Workers.
  • Coordinate and monitor clinical operations and patient care delivery.
  • Conduct case conferences and assist with care planning and coordination.
  • Review clinical documentation, OASIS assessments, and physician orders for accuracy and compliance.
  • Monitor patient outcomes, clinician productivity, and quality indicators.
  • Provide education, coaching, and mentoring to clinical staff.
  • Assist with quality assurance activities and survey readiness initiatives.
  • Serve as a clinical resource to staff, patients, physicians, and referral sources.
  • Promote teamwork, accountability, and exceptional customer service.
  • Assist in developing and implementing clinical policies and procedures.

Qualifications

  • Current Illinois Registered Nurse (RN) license in good standing.
  • Minimum of two years of supervisory or management experience in a Medicare-certified Home Health Agency.
  • Strong knowledge of Medicare Conditions of Participation and home health regulations.
  • OASIS knowledge and documentation experience.
  • Excellent clinical assessment and critical-thinking skills.
  • Strong leadership, communication, and organizational abilities.
  • Excellent computer and electronic medical record proficiency.
  • Ability to manage multiple priorities in a fast-paced environment.
