Tranquil Harmony Counseling is seeking a compassionate and driven Licensed Clinical Social Worker (LCSW) to join our dedicated team in Rockford, Illinois. This full-time position is fully remote, enabling our social workers to provide vital support to clients in a community health clinic setting. The primary goal of this role is to assist individuals facing major mental illnesses and emotional challenges, providing them with the resources and interventions necessary for recovery and well-being. This position offers a competitive salary ranging from USD 70,000 to 85,000 annually.

The position is structured to allow for remote work flexibility that meets the needs of both the staff and our clients. While much of the work occurs from home, our Licensed Clinical Social Worker will frequently engage with community members and other professionals through telehealth methods. • Engage directly with clients in both individual and group therapy sessions, providing assessments, treatment planning, and specialized interventions tailored to the unique needs of clients facing mental health challenges.
• Develop, implement, and manage integrated care plans that facilitate continuity of care by coordinating with family members, healthcare providers, and community resources.
• Conduct initial screenings and ongoing assessments to tailor therapeutic approaches; this includes a focus on evidence-based practices that align with developmental and cultural competency.
• Deliver crisis intervention services, supporting clients in acute distress and managing emergency situations effectively and calmly.
• Actively participate in team collaborative meetings, driving projects aimed at enhancing treatment quality and service delivery, while sharing insights and feedback to aid in the overall improvement of clinic protocols.
• Monitor client progress and outcomes, ensuring timely documentation and submission of reports for both internal quality assurance and regulatory compliance.
• Provide on-call services during and after program hours, ensuring that support is always available to clients in need.

The ideal candidate will work with a diverse range of clients, primarily those facing significant mental health issues such as severe emotional disorders and major mental illnesses. This role requires understanding and sensitivity to the cultural backgrounds, beliefs, and needs of various client populations, ensuring that all interactions promote trust and respect.

• A relevant Bachelor’s Degree in Social Work, Psychology, or a related field is mandatory. A Master’s Degree or higher is preferred.
• Licensure as a Clinical Social Worker (LCSW) is required for this position.
• Minimum of 2 years of experience in a clinical or community-based setting, with proven engagement with underserved populations.
• In-depth knowledge of mental health frameworks, treatment modalities, and ethical guidelines in social work practice.

• Experience working within community health clinics or related settings.
• Familiarity with integrated care models and trauma-informed care approaches.
• Demonstrated history of working effectively within multidisciplinary teams.
