Clinic Registered Nurse (RN) - Escondido, CAVivo HealthStaff is recruiting for a Clinic Registered Nurse (RN) in Escondido, CA. This is a permanent position with salary and benefits.The Clinic RN is responsible for coordinating and providing direct and indirect nursing care to patients at the clinic as well as in their homes. This role works in close coordination with the Physician, clinic staff, and interdisciplinary team (IDT) members to manage ongoing care, address complex clinical situations, and support the operation of the primary care clinic. The Clinic RN collaborates effectively with colleagues and stakeholders to promote team culture and the mission of the organization.Essential FunctionsConduct clinic nursing assessments to determine the care needs, preferences, and goals of patients and actively participate in IDT meetings to develop care plans.Visit patients' homes to deliver nursing interventions as needed.Deliver and document nursing interventions as agreed upon in the patients' care plans, including but not limited to maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments.Remain alert to input from team members, patients, and caregivers and update the IDT promptly regarding any changes in patients' conditions or medical status.Work with the Physician and other IDT members to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).Instruct patients, families, and caregivers on disease processes, self-care techniques, and prevention strategies.Support the enrollment of prospective patients into the program in coordination with the marketing team.Participate in end-of-life care coordination and support.Provide backup coverage to other clinic nursing functions as needed.Complete all required documentation in a timely and accurate manner.Protect privacy and maintain confidentiality of company procedures, as well as patient and employee information.Follow clinic policies and procedures and participate in quality improvement activities, staff training, and meetings.Perform other duties as assigned.Education and Certification RequirementsGraduate of an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.Unencumbered California Registered Nurse (RN) License.Current Basic Life Support (BLS) / First Aid Certification required.Valid driver's license with proof of auto insurance.Skills & ExperienceMinimum of two years of nursing experience in a clinical setting with a frail or elderly population.Nursing knowledge and skills necessary to treat frail, elderly patients and manage complex clinical situations.Highly motivated, self-directed, and able to execute tasks in a quickly changing environment.Ability to make sound decisions in emergency situations.Strong clinical, organizational, and communication skills when working with seniors, their families, and interdisciplinary team members.Experience and competency working with people from diverse backgrounds.Commitment to providing high-quality care to vulnerable senior populations.Bilingual Spanish-English preferred.About Vivo HealthStaffVivo HealthStaff is a healthcare consulting company specializing in clinical staffing, clinical program development and operations, healthcare consulting, and telemedicine implementation.
