Description:
POSITION PURPOSE
The Specialty Pharmacy Pharmacist works according to legal and ethical guidelines to ensure the correct and safe supply of medical products to Specialty Pharmacy patients. They are also involved in maintaining and improving patient health by providing pharmaceutical care services including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. The pharmacist also ensures compliance with Federal, State and local laws and regulations as well as proper medication storage and documentation.
ESSENTIAL FUNCTIONS
Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
Receives and reviews all prescriptions for accuracy and clinical appropriateness. Contacts prescribers for clarification regarding prescription elements, clinical appropriateness, and inquiries. Identifies and responds to medication issues associated with prescriptions being dispensed.
Prepares, labels, and dispenses medications for patients in accordance with physicians' orders and ensure compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and control substances and the maintenance or required documentations.
Performs drug utilization reviews. Identifies and documents utilization management and appropriateness of therapy.
Reviews EMR, recent medical history, patient labs, height, and weight when needed for therapy.
Completes ADR/FDA Med Watch Form for all reported ADRs and Compliant Form within specified time per policy and procedure.
Conducts patient education, interventions, and therapy assessments: documents patient care plans; maintains related medical documentation; and communicates to physician office according to policy.
Answers questions and provides information pertaining to drug interactions, side effects, dosage, administration, and medication storage.
Maintains security of pharmacy, PHI, controlled substances, and disposal of medications.
Maintains organization and efficiency in fulfillment workflow and management of inventory. Verifies correct product, supplies, and educational materials for dispensed orders.
Maintains pharmacy records, including shipping logs, temperature logs, inventory logs, prescription hardcopy, and other documentation as required by bop or contractual requirements.
Complies with quality metrics, performance criteria, turnaround times, and customer service metrics
Completes and maintains all required documentation for patient management and reporting. Reviews accuracy of prescription data entry, order filling, and shipping. Reviews and support prior authorization completion and medical documentation.
Participates in quality program initiatives and improvement activities.
Assists in training and supervision of pharmacy personnel.
Maintains competency in area of practice by attending seminars, obtaining continuing education credits and successfully completing all clinically generated disease state overviews
Assists with workflow management and address any workflow backlogs.
Complies with all regulatory requirements, including HIPAA/HITECH.
Other responsibilities as assigned by the manager.
Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of
Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
MINIMUM QUALIFICATIONS
Must possess a comprehensive knowledge of the pharmaceutical industry or specialty pharmacy as normally obtained through a Bachelor's degree or Doctorate of Pharmacy and 3 years of pharmacy experience.
Licensed as a registered pharmacists in required states including non-resident licensures as required for business operations.
Knowledge of Specialty medications is preferred.
Knowledge of home infusion, mail order pharmacy or general pharmacy operations is preferred.
Knowledge of Microsoft office products, including Word, Excel, PowerPoint and other graphics or presentation software.
Must be comfortable operating in a collaborative, shared leadership environment.
Must possess a personal presence characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
Must be able to set and organize own work priorities, and adapt to them as they change frequently. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles.
Must possess the ability to comply with Trinity Health policies and procedures.
Frequent immobility and/or sitting required for extended periods of time.
Some bending and stooping required.
Requires manual dexterity to operate keyboard, calculator, photocopy machine, and other office equipment.
Requires eyesight correctable to 20/20 to read reports and computer terminals.
Requires hearing within normal range for telephone use.
Work in a fast-paced production environment.
Normal office environment and/or standing for prolong periods of time.
Occasional high stress work.
Pay Range- $57.90/hr- $95.54/hr
