Work Environment

Key Responsibilities

• Engage in collaborative efforts with a multi-disciplinary team—comprising physicians, nurses, and therapists—to evaluate client needs and develop integrated care plans.
• Design and implement individualized care strategies for hospice patients and their families, ensuring considerations of emotional, social, and spiritual dimensions of care.
• Provide education and resources about mental health services, hospice processes, and available community supports to patients and their families.
• Remain current with industry best practices and regulatory requirements to deliver informed and effective support, contributing to ongoing training and staff development initiatives.
• Document all interactions and interventions in our electronic health record (EHR) system to facilitate continuity of care across the team.

Patient Population / Client Focus

This role specifically addresses the needs of elderly patients and their families facing terminal illnesses, ensuring they feel respected, understood, and cared for during a sensitive time in their lives. Your expertise will help navigate complex emotional landscapes and foster healthy coping mechanisms for all involved, creating a compassionate environment where patients receive holistic and comprehensive support.

Qualifications

• Master’s degree in Social Work from an accredited institution required.
• Current and valid LCSW license in the state of Nevada.
• Strong clinical and interpersonal skills aimed at effectively working with patients and families.
• Proficient in using technology for patient engagement and documentation, with a willingness to adopt new tools as needed.

Preferred Experience

• A minimum of 2 years of experience as a clinical social worker, preferably in a healthcare or hospice setting.

Compensation and Benefits

This role offers a competitive salary range of $100,000 – $110,000 annually, commensurate with experience and qualifications. In addition, our comprehensive benefits package includes:
