Work Environment

As a remote team member, you will be part of a collaborative digital workspace where connection and support are emphasized. Key Responsibilities
  • Engage in collaborative efforts with a multi-disciplinary teamโ€”comprising physicians, nurses, and therapistsโ€”to evaluate client needs and develop integrated care plans.
  • Design and implement individualized care strategies for hospice patients and their families, ensuring considerations of emotional, social, and spiritual dimensions of care.
  • Provide education and resources about mental health services, hospice processes, and available community supports to patients and their families.
  • Remain current with industry best practices and regulatory requirements to deliver informed and effective support, contributing to ongoing training and staff development initiatives.
  • Document all interactions and interventions in our electronic health record (EHR) system to facilitate continuity of care across the team.
Patient Population / Client Focus

Qualifications
  • Masterโ€™s degree in Social Work from an accredited institution required.
  • Current and valid LCSW license in the state of Nevada.

Compensation and Benefits
