Social Work Supervisor (LMSW)
Position Overview

We are seeking an experienced and compassionate Social Work Supervisor (LMSW) to provide leadership, clinical oversight, and supervision to a case management team serving vulnerable populations. This role is responsible for ensuring the delivery of high-quality, client-centered services while maintaining compliance with agency, regulatory, and funding requirements.

The ideal candidate will possess strong supervisory experience, excellent clinical judgment, and a commitment to supporting individuals impacted by HIV/AIDS, serious mental illness, substance use disorders, homelessness, and other complex social challenges.

Key Responsibilities
Clinical Supervision & Program Oversight
  • Provide clinical oversight and supervision of case management services to ensure quality care and compliance with agency and government regulations.
  • Assign cases to case managers and prioritize workloads based on client needs and program requirements.
  • Review and supervise comprehensive service plans, ensuring appropriate goal setting, implementation, and ongoing reassessment.
  • Ensure initial assessments, psychosocial evaluations, and medical assessments are completed accurately and timely.
  • Oversee mental health, substance use, medical, benefits, and supportive service referrals that promote independent living and overall client well-being.
  • Support staff in addressing complex client situations, conflict resolution, and service engagement challenges.

Staff Leadership & Development
  • Conduct weekly individual supervision meetings with case management staff to review client progress, documentation, and service delivery.
  • Provide onboarding, training, coaching, mentorship, and ongoing professional development to staff.
  • Complete probationary and annual performance evaluations.
  • Identify staffing needs, performance concerns, and service trends and communicate recommendations to program leadership.
  • Serve as Manager-in-Charge in the absence of the Program Director, as assigned.

Quality Assurance & Compliance
  • Conduct regular chart reviews and audits to ensure documentation is accurate, complete, and compliant.
  • Monitor service plans, reassessments, progress notes, and psychosocial assessments for quality and timeliness.
  • Ensure confidentiality standards and privacy regulations are consistently followed.
  • Monitor compliance with home visit policies and documentation requirements.
  • Maintain supervision records and prepare required reports, statistics, and program documentation.

Program Operations & Housing Coordination
  • Coordinate with HASA and other referral sources to facilitate client intake, placement, and service engagement.
  • Provide weekly updates to the Program Director regarding referrals, placements, occupancy rates, and program outcomes.
  • Collaborate with case management and maintenance staff to maintain a minimum monthly occupancy rate of 95%.
  • Support timely placement of new clients into available units in accordance with program guidelines.
  • Work closely with leadership to ensure safe, appropriate living environments for residents and address housing-related concerns.
  • Assist with the development and implementation of housing maintenance and occupancy guidelines.

Community Partnerships
  • Develop and maintain collaborative relationships with community-based organizations, healthcare providers, and ancillary service agencies.
  • Participate in staff meetings, community meetings, and interdisciplinary case conferences as required.
  • Facilitate coordinated care efforts to support positive client outcomes.

Required Qualifications
Education & Licensure
  • Master's Degree in Social Work (MSW) required.
  • Current New York State Licensed Master Social Worker (LMSW) required.

Experience
  • Minimum of four (4) years of supervisory experience managing a case management team.
  • Experience providing individual, family, and group counseling.
  • Demonstrated experience working with individuals affected by HIV/AIDS, serious mental illness, substance use disorders, homelessness, and other complex psychosocial needs.
  • Strong knowledge of case management principles, service planning, documentation standards, and community resources.

Preferred Qualifications
  • Experience working within HASA and/or supportive housing programs, including congregate or scattered-site models.
  • Bilingual/Bicultural Spanish-speaking preferred.
  • HIV/AIDS Pre-Test and Post-Test Counseling Certification preferred.
