- Conduct virtual clinical assessments for post-discharge and follow-up visits.
- Complete medication reconciliation and update patient care plans.
- Provide real-time clinical support to EMTs during in-home patient assessments.
- Provide clinical oversight for tasks delegated to EMTs, CHWs, and Social Workers.
- Address clinical quality gaps within NP scope of practice.
- Coordinate follow-up appointments, referrals, and diagnostic testing.
- Communicate with hospitals, primary care providers, and community care partners as needed.
- Participate in interdisciplinary discussions and case reviews.
- Document clinical findings in accordance with CMS, state, and accreditation requirements.
How you qualify
Required
- Active Arizona Nurse Practitioner license.
- Five or more years of clinical experience in primary care or internal medicine.
- Outpatient or community-based experience - home visits or clinic settings.
- Ability to support field clinicians remotely and in real time.
- AHCCCS registration.
- Experience in telehealth or transitions-of-care settings.
- Experience managing patients with multiple chronic conditions.
- Familiarity with HCC coding or HEDIS measures.
- Experience conducting annual comprehensive visits.
