Calhoun Families First Center is seeking a compassionate and skilled Full-Time Clinical Social Worker to join our hospice team as we provide essential care for patients and their families during challenging times. This position is primarily remote, allowing our Clinical Social Worker the flexibility to work from home while visiting patients at our hospice center as needed. In this vital role, you will:

• Conduct thorough psychosocial assessments to gather comprehensive insights about patients’ emotional, social, and financial challenges.
• Engage with interdisciplinary medical teams to collaboratively develop and implement holistic care plans that address the psychosocial aspect of patient health.
• Provide individual and group counseling focused on adjustment to illness, grief, and coping mechanisms, tailored to the unique circumstances of each patient.
• Coordinate with local community organizations to establish post-discharge services aimed at providing ongoing support and ensuring seamless care transitions.
• Act as an advocate for patients and families, offering education on available treatment options and resources to help navigate the healthcare system.
• Maintain meticulous documentation of all patient interactions and care plans in compliance with hospital policies and legal requirements.
• Be available for crisis intervention and provide timely emotional support to patients and their families during moments of distress.
• Participate in hospital committees and contribute your expertise on social work-related topics to enhance organizational practices.

Your work will center on assisting patients facing terminal illnesses, providing them and their families with empathy and understanding. You will support those experiencing profound challenges while ensuring their dignity and quality of life are prioritized during end-of-life care.

Applicants must possess a Master’s degree in Social Work from an accredited program and hold an active state license as a Licensed Clinical Social Worker (LCSW). We seek professionals who can demonstrate strength in communication, problem-solving, and collaboration.

While not mandatory, candidates with at least two years of experience in a healthcare or hospital setting will be favored. Familiarity with medical terminology and mental health interventions, along with demonstrated skills in crisis management, are highly desirable.

This position offers a competitive annual salary between USD 70,000 and USD 100,000, commensurate with experience and qualifications, along with a sign-on bonus. Our benefits package includes:
