Position: Social Work Assistant

Schedule: 16 hours or more

Reports To: Director of Social Services

The Social Worker is responsible for implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record. The Social Worker is also responsible for collaborating with the nursing, MDS, Admissions and rehabilitation departments to facilitate discharge plans for residents. The Social Worker is responsible for informing and educating the staff/resident/family/responsible party of the resident’s personal and property rights, as well as his/her rights related to health services as pertaining to the resident. This position also maintains updated resources for residents and families in regard to community resources and services, as well as service agencies and organizations. Maintains a record of all reported concerns, assists in resolving them and assures appropriate follow up and documentation. The Social Services is easily accessible to residents, family and staff. Provides timely referrals to appropriate agencies for assistance to the resident/family (i.e. conservatorship application, Medicaid application, etc).

Key Responsibilities:

  • Craft and document personalized care plans for residents' social and emotional well-being.

  • Collaborate with various departments to ensure effective discharge plans.

  • Knows and complies with Residents’ Bill of Rights.

  • Maintain an updated database of community resources for residents and families.

  • Address reported concerns, provide resolutions, and ensure proper documentation.

  • Extend accessibility to residents, families, and staff for support and guidance.

  • Offer timely referrals to appropriate agencies for necessary assistance.


Preferred:

  • Bachelor’s Degree from an accredited program in Social Work

Requirements:

  • Bachelor’s Degree in Human Services or related field.

  • Must be knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as be familiar with residents’ rights and documentation requirements in accordance with current regulations and/or guidelines.

  • Attention to detail, good follow through skills and ability to prioritize multiple tasks.

  • Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus.

  • Must have a vehicle and a valid drivers’ license.

  • Demonstrates cooperation in assisting others and ability to work as a team.

  • Patience, tact, enthusiasm and a positive attitude toward the residents and their support networks.

  • Ability to remain calm in stressful situations.

  • Ability to communicate with residents, staff and family members in a professional manner.
