Job description

Position Summary

LifeSource Hospice is seeking a compassionate and experienced RN Hospice Case Manager to join our interdisciplinary team serving patients in Bergen County, NJ. This role is responsible for managing a caseload of hospice patients, coordinating care across disciplines, and ensuring compliance with hospice regulations and standards.

The ideal candidate has hospice, home health, or long-term care experience and is comfortable working independently in the field.

Key Responsibilities
  • Manage a hospice caseload of approximately 15 patients

  • Conduct comprehensive nursing assessments and develop individualized plans of care

  • Provide skilled nursing visits and symptom management

  • Coordinate care with physicians, social workers, chaplains, aides, and other IDT members

  • Ensure documentation meets CHAP standards and regulatory requirements

  • Utilize WellSky EMR for timely and accurate charting

  • Perform medication reconciliation and coordinate e-prescribing via BetterRx

  • Provide patient and family education related to disease progression and end-of-life care

  • Participate in IDT meetings and care planning

  • Collaborate with facilities and community partners

  • Travel to patient homes and facilities within Bergen County

    Qualifications

    • Active NJ RN license in good standing

    • Minimum 1 year experience in hospice, home health, or long-term care

    • Familiarity with CHAP hospice standards preferred

    • Experience with WellSky EMR and BetterRx a plus

    • Strong clinical assessment and communication skills

    • Compassionate, patient-centered approach

    • Reliable transportation and valid driver’s license

    • Ability to work independently and manage time effectively

    Schedule
    • Full-time

    • Monday–Friday

    • On-call rotation (one weekend per month in Bergen county vicinity)
