What You’ll Do
  • Performs Annual Wellness Visits, develops preventive care plans, and documents per CMS and provider guidelines.
  • Manages care for high-risk and ACO patients, coordinating services and participating in daily/multidisciplinary huddles.
  • Provides direct nursing care including triage, treatments, injections, labs, and patient admissions per protocol.
  • Maintains compliance with licensure, safety, infection control, and quality improvement standards.
  • Collaborates across departments, supports organizational mission, and upholds CommWell Health’s core values.
What We’re Looking For

Required

  • BLS Certificate from American Red Cross or American Heart Association
  • Registered Nurse (RN) with current NC license
  • Minimum One (1) year experience as RN, LPN, or other related medical field
  • Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges

Preferred

  •  Prefer experience with Care Management, Value Based Care, Population Health, Medicare Annual Wellness.
