: Fort Wayne, IN 46809.
Job Type:
Independent Contractor (1099).
Pay Rate:
$130/hr onsite patient care hours, $100/hr telemedicine lab review hours.
Schedule:
8/11, 8/12, 9/22, 9/23. 8am-5pm.

Full Description:
A workplace wellness screening program is seeking a Spanish Speaking Provider to provide onsite preventive wellness services within a professional workplace environment in Fort Wayne, IN. This role focuses on preventive care, health screenings, and wellness consultations delivered during scheduled clinic events.

Providers typically see up to 20 patients per clinic day and must be comfortable practicing autonomously while making independent clinical decisions within their scope of practice. Clinic events are scheduled periodically based on workplace wellness programming and are not part of a fixed weekly schedule. Administrative coordination is provided for scheduling and clinic logistics.

Responsibilities:

  • Set up and prepare the mobile clinic space within corporate environments to support employee wellness initiatives.
  • Conduct general wellness examinations.
  • Review patient health history and current concerns.
  • Provide preventive care guidance and health education.
  • Perform blood draws (venipunctures).
  • Refer patients for follow-up care when appropriate.
  • Review lab results via telehealth consultations.
  • Document encounters according to clinical standards.

Requirements:

  • Must hold an active, unrestricted Nurse Practitioner / Physician Assistant license in Indiana.
  • Certification as FNP, FNP-C, FNP-BC, or ACNP.
  • Active basic life support (BLS/ ACLS) CPR certification required.
  • A collaborating physician is necessary unless state requirements for independent/autonomous practice are met.
  • Must be bilingual fluent in Spanish.
  • Background in internal medicine, family practice, urgent care, or emergency room preferred.
  • Must be able to perform phlebotomy.
  • Proficient with computers, customer service-oriented, and able to work independently.
  • Focused on preventive medicine and patient education.
  • Must bring a personal laptop or tablet to each shift to complete electronic charting and documentation.
  • Maintain patient confidentiality and professional care standards.
  • Must be able to reliably commute to Fort Wayne, IN 46809.
