Position Overview

Well Spring Healthcare Services is seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position dedicated to improving the mental and emotional well-being of seniors and their families. In this vital role, you will assess the psychosocial needs of clients, develop tailored care plans, and facilitate access to critical community resources. You will be part of a dedicated team that prioritizes personal growth, healing, and empowerment for individuals facing mental health challenges, particularly anxiety and depression.

Work Environment

This position is primarily remote, allowing for flexibility and continuity in care while working from the comfort of your own home. Although full-time hours are scheduled from Monday to Friday, 8:00 AM to 4:30 PM, there may be occasional evening or weekend hours required based on client needs. On-call availability may also be necessary for urgent situations. We value a flexible approach to scheduling to best serve our clients and ensure that they receive uninterrupted support.

Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial well-being, evaluating emotional, mental, and social factors that affect their quality of life.
  • Collaborate with clients and their families to develop individualized care plans that align with their specific needs, goals, and preferences.
  • Provide counseling and emotional support to seniors and their families, helping them navigate life changes, health challenges, and related stressors.
  • Advocate for clients' rights and facilitate access to essential community resources, ensuring they receive necessary support services.
  • Work closely with interdisciplinary teams, including healthcare professionals, nurses, and caregivers, to enhance the overall quality of care provided to clients.
  • Regularly visit clients to monitor their well-being, assess living conditions, address any immediate concerns, and ensure safety.
  • Facilitate family communication and conflict resolution for those involved in the care of senior clients, promoting collaborative approaches to care.
  • Educate clients and their families about available community programs, support groups, and resources that can enrich their social engagement and overall well-being.
  • Maintain accurate and organized documentation of client assessments, care plans, progress notes, and any interactions to ensure comprehensive care continuity.
  • Stay informed on current social work practices, policies, and ethical standards, ensuring compliance with relevant regulations.
Patient Population / Client Focus

You will primarily be serving seniors, a population that often faces unique emotional and mental health challenges. Your role will be crucial in providing support to those experiencing anxiety and depression, helping them gain empowerment and control over their emotional landscapes. Your efforts will directly contribute to improving client outcomes, enhancing their quality of life, and promoting greater independence.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Current and valid LCSW license in the state of Arizona.
  • A minimum of two years of post-licensure experience in a social work capacity, ideally within a community health or geriatric setting.
  • In-depth knowledge of social work principles, familiar with aging-related issues, and strong awareness of available community resources.
  • Exceptional assessment, counseling, and problem-solving skills are paramount.
  • Strong interpersonal skills, with the ability to empathize and build rapport with seniors and their families.
  • Proficient in electronic health record (EHR) systems and other relevant computer applications.
Preferred Experience
  • Experience working in a community health clinic or similar setting.
  • Familiarity with interdisciplinary care models, particularly in geriatric services.
  • Expertise in navigating community resources and support services for seniors.
Compensation and Benefits

This position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
