Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Travel Pay Package: Up to $2,300.00 weekly.

    *(If your home location is more than 50 miles); $1540.00 weekly pay will be paid in the form of non-taxable stipend.

Location: Milwaukee, WI, 53226; Coverage area: Local within 50-60 miles radius.

Shift: 8:00 Hours/Day, 09:00 AM to 05:00 PM

Job Summary:

We are seeking an experienced Registered Nurse (RN) to provide skilled nursing care to Medicare Home Health patients in their homes. The RN will deliver care in accordance with physician orders and individualized Plans of Care while collaborating with RN Case Managers, Clinical Managers, physicians, patients, families, and the interdisciplinary care team.

The ideal candidate will have strong home health and bedside nursing experience, excellent clinical judgment, and the ability to independently manage multiple patients throughout the day.

Key Responsibilities

  • Provide skilled nursing care to patients in their individual home environments according to the Plan of Care and physician orders.
  • Manage a daily caseload of approximately 6–8 patients.
  • Conduct initial and ongoing comprehensive assessments of patient needs, including OASIS assessments when required.
  • Assess and evaluate patients' level of care, needs, and clinical requirements.
  • Implement individualized Plans of Care and recommend revisions based on patient progress and changing needs.
  • Evaluate patient outcomes and response to interventions.
  • Educate patients and family members regarding disease processes, medications, self-care techniques, prevention strategies, and community resources.
  • Provide symptom management, crisis intervention, family support, and specialized nursing care as needed.
  • Provide palliative and end-of-life care when applicable.
  • Assess caregiver ability to meet patient needs and provide appropriate education and support.
  • Supervise LPN/LVNs, home health aides, and paraprofessionals in accordance with regulatory requirements.
  • Communicate patient status, changes in condition, and care outcomes to physicians and other members of the healthcare team.
  • Collaborate regularly with RN Case Managers, supervisors, and the interdisciplinary team.
  • Participate in interdisciplinary team meetings to coordinate care, exchange information, and address patient needs.
  • Maintain accurate and timely clinical documentation and patient medical records.
  • Prepare clinical and progress notes demonstrating progress toward established goals.
  • Participate in agency quality assessment and performance improvement (QAPI) programs.
  • Participate in assigned on-call coverage.
  • Meet or exceed established productivity standards.
  • Follow agency policies and applicable federal and state regulations.
  • Use company resources responsibly and contribute to community education regarding available services.
  • Perform other appropriate duties and assignments as directed.

Required Qualifications

  • Active RN license in the state of Wisconsin and in good standing – Required
  • 2+ years of RN experience – Required

  • Current driver's license – Required
  • Current automobile insurance – Required
  • Ability to travel up to 60 miles from the branch – Required
  • Ability to manage multiple home health visits independently.
  • Ability to use a tablet for clinical documentation – Required.
