Weekend On-Call Registered Nurse

After-hours support. Mostly remote. Critical impact.

Join a compassionate team dedicated to providing comfort, support, and responsive end-of-life care when patients and families need it most. As a Weekend On-Call Registered Nurse, you will provide skilled nursing support, symptom management, and crisis intervention to hospice patients receiving care in homes and facilities throughout the Cleveland area. This role offers a unique blend of independence, flexibility, and meaningful patient impact while working closely with an experienced interdisciplinary team.

Schedule, Hours & Territory

• Dedicated Weekend On-Call RN Position
• Coverage Hours: Friday at 4:30 PM through Monday at 8:00 AM
• Territory: Cleveland, OH and surrounding service area
• Combination of scheduled patient visits and on-call responsibilities
• Primarily remote when not actively responding to patient needs in the field

Key Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs by providing timely nursing assessments, interventions, and clinical support.

• Perform on-call visits to assess changes in patient condition, manage symptoms, and implement appropriate care plans.

• Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members to ensure continuity of care.

• Provide education and guidance to patients, families, and caregivers regarding medications, symptom management, and end-of-life care.

• Facilitate admissions, deaths, and other urgent clinical situations in accordance with hospice policies and regulatory requirements.

• Complete accurate and timely documentation of patient assessments, interventions, and care coordination activities in the EMR.

• Deliver compassionate, patient-centered care while supporting the physical, emotional, and psychosocial needs of patients and families.

Qualifications

• Active Registered Nurse (RN) license in the state of practice required.

• Minimum of one (1) year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred.

• Strong clinical assessment, symptom management, and critical thinking skills.

• Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field.

• Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred.

• Excellent communication and interpersonal skills with the ability to effectively support patients, families, caregivers, and healthcare professionals.

• Valid driver's license, reliable transportation, and current automobile insurance required.
