Job description

Provides in-home services to our patients.

  • Provides care ordered by the physician as indicated by the plan of care to all patients on caseload including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding, and transferring patient.
  • Takes accurate vital signs.
  • Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities.
  • Participates in case conferences (IDG's) to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Notifies appropriate licensed staff when patient complains of pain.
  • Positions patient in bed with proper body alignment.
  • Assists patient with ambulation as necessary.
  • Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Supports cultural practices as long as these practices do not harm the patient/family or interfere with the planned course of medical treatment.
  • Identifies and communicates findings to the Case Manager and/or Clinical Manager.
  • Participates in the Agency's other mandated activities, as requested by Clinical Manager or Director of Patient Care Services (DPCS).
  • Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and Agency policy and procedure.
  • Identifies and communicates findings to the supervising RN.
  • Works as part of the interdisciplinary group.
  • Understands regulations/standards applicable to Hospice.
  • Thorough knowledge and understanding of the functions of a Hospice Agency.
  • Demonstrates knowledge of the dying patient and pain control measures.
  • Exhibits genuine interest in and compassion for patients and families dealing with end-of-life issues.
  • Understands hospice philosophy and issues of death and dying.
  • Ability to be flexible, organized, and function under stressful situations.
  • Able to communicate effectively in English, both verbally and in writing.
  • Excellent interpersonal skills.
  • Excellent writing and presentation skills.
  • Knowledge of general modalities and scope of practice within the state of Agency operation.
  • Candidate should be self-directed and can work in the field with minimum supervision.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
