As a Licensed Vocational Nurse (LVN) at Momentum, you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our new state of the art Adult Day Program (ADP). 

Licensed Vocational Nurse (LVN) Essential Responsibilities: 

  • Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
  • Implement and oversee the health care plans for residents
  • Administer medications and perform medical procedures as needed
  • Collaborate with a multi-disciplinary team to ensure the well-being of residents
  • Maintain accurate medical records and documentation
  • Offer support and guidance to Direct Support Professionals (DSP)
  • Participate in complex case management and engage in continuous learning opportunities

Licensed Vocational Nurse (LVN) Qualifications:

  • Valid License Vocational Nurse (LVN) license in the state of California
  • CPR certification
  • Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals 
  • Strong communication and interpersonal skills
  • A genuine passion for providing quality care and support to individuals with disabilities
  • Ability to work effectively in a team-oriented environment
  • Willingness to adapt to the needs of the residents and facility
