IMMEDIATE SUPERVISOR: Nurse Manager

SUPERVISORY RESPONSIBILITIIES: None

FLEXIBLE WORK: Hybrid (80% on-site, 20% remote)

SALARY: LPN: Band D ($31.00 - 39.00/hour); RN: Band E ($34.50 - $45.00/hour)

The mission of Charles River Community Health (CRCH) is to partner with individuals and families so they can thrive and lead healthier lives by delivering the comprehensive, integrated, and equitable primary healthcare that matters most to them. The Staff Nurse provides direct professional nursing care and education to Health Center patients and ensures continuity and quality of care. The Staff Nurse has a solid understanding of patient needs and will work collaboratively with primary care providers and a multidisciplinary team to develop, assess, coordinate, and optimize patient care plans.


YOUR RESPONSIBILITIES

  • Function effectively and safely within the scope of practice of the Licensed Practical Nurse/Registered Nurse to strengthen the practice and provide patient-centered care, working to top of license.
  • Provide culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.
  • Triage all calls and assist with walk-ins. When organizing care, set priorities based upon acuity.
  • Maintain awareness of clinic flow and re-arrange provider schedules to accommodate urgent care needs.
  • Demonstrate knowledge of community resources and assist staff, patients, and families to effectively utilize them. Ensure effective coordination of care across the care continuum, including home care, outpatient clinics, emergency/urgent care, and hospital settings.
  • Facilitate the advanced care planning process, assisting patients and families to consider options and goals for future healthcare decisions. Document advanced care goals in the EHR.
  • Serve as a resource person to other members of the team around the care needs of patients and provide support to other members of the clinical and administrative team in an accessible, open manner.
  • Delegate safely and effectively to support staff tasks that are within scope of practice.
  • Communicate effectively and clearly with members of the care team regarding patient status and needs.
  • Participate in the establishment and documentation of standard triage protocols, procedures, and patient education information, and present materials for review/approval by medical leadership.
  • In conjunction with the nursing leadership, establish and document procedures for treatment room stocking and securing clinical supplies; ensure supplies are properly stored to enhance efficiency.
  • Work collaboratively with other Health Center staff in meeting JCAHO and other regulatory and/or funding requirements.
  • Perform other duties as required by Nurse Manager, Director of Nursing, or designee.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

  • Bachelor of Nursing preferred.
  • Must have Massachusetts LPN or RN license.
  • Excellent communication and documentation skills required.
  • Certification in CPR required.
  • Must have knowledge base including basic health teaching, family planning education, and how to care for common pediatric and adult illnesses.
  • Must be willing to educate and mentor clinical support staff as needed.
  • Must be willing to work one evening per week and one Saturday per month.
  • Must have basic computer skills preferred or willingness to develop these skills.
  • Willingness to support training needs for students and internships as requested.
  • Experience with telephone assessment and crisis management preferred.
  • Bilingual (Spanish/English, Portuguese/English) preferred.
  • Must have a willingness to work flexible hours to meet the organization's needs/demands.
  • Must be able to travel to either Charles River Community Health site (Brighton and Waltham) as needed.
  • Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
  • Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
