Weekend Hospice Social Worker

Agency: Puget Sound Hospice of King County
Location: King County
Pay: $40-50 an hour, DOE
Schedule: Sat and Sun, 8 hours each day in patient's home

What You'll Do
As a Hospice Social Worker, you'll play a vital role in helping patients and families navigate the emotional, social, and practical challenges that come with serious illness and end-of-life care in a home setting.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments and develop individualized care plans for hospice patients.
  • Provide clinical counseling and emotional support to patients and families, addressing social, emotional, behavioral, and spiritual concerns.
  • Educate patients and families about hospice services, end-of-life planning, advance directives, and community resources.
  • Offer guidance on behavioral health issues and provide crisis intervention when necessary.
  • Advocate for patient needs, ensuring their rights are respected and upheld within the healthcare system.
  • Collaborate with the interdisciplinary hospice team to coordinate holistic care and ensure continuity of services.
  • Maintain accurate, timely, and confidential documentation in compliance with HIPAA and organizational standards.

Qualifications:
