Hospice of the East Bay (HEB) is a community-based nonprofit hospice and palliative care organization that has been serving patients and families with compassion and dignity for nearly 50 years. We provide hospice and palliative care in patients' homes and in our six-bed inpatient hospice facility, ensuring every patient receives exceptional care wherever they are most comfortable. Our interdisciplinary team works together to support not only our patients but also their loved ones, offering bereavement services for a minimum of one year after a loss. Through our sponsored care program, we remain committed to providing care regardless of a patient's ability to pay.

Position Summary:
Reporting to the IDT Manager, the Visit Nurse is responsible for making home visits, identifying and coordinating patient/family care needs under the direction of the attending physician, Medical Director and other members of the interdisciplinary group, and in conjunction with the Nurse Case Managers.
ESSENTIAL FUNCTIONS:
  • Performs the essential functions of the job
  • Provide direct hospice nursing care to patients and families
  • Assess patients and report to the attending physician and other appropriate interdisciplinary group members, reactions to treatments, medications and changes in the patient's physical or emotional condition
  • Attend and participate in nursing staff meetings
  • Supervise Home Health Aides in the home as necessary
  • Communicates with the interdisciplinary group members as appropriate regarding changes in patient's condition and care plan
  • Coordinate care with other agencies, suppliers and facilities participating in patient care in conjunction with Nurse Case Manager
  • Coordinate patient care with awareness of insurance benefits, necessary authorizations and cost containment
  • Maintain necessary records and reports in a timely manner
  • Participate in training new staff members
  • Participate in Quality Improvement activities
  • Coordinate time off with IDT Managers, taking into consideration adequate staffing levels
  • Adheres to National Patient Safety Goals.
  • Completes and submits patient-related documentation per agency policy.
  • Responsible for following the Agency's process for effective "hand off" communication.

POSITION REQUIREMENTS:
Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements.
Ability to assess and respond to needs of patients and families in the home setting. Must be skilled in nursing practice, including IV skills. Must be able to cope with family, emotional stress and be tolerant of individual lifestyles without judgment. Must have ability to establish and maintain cooperative relationships with professionals and lay groups. Must demonstrate ability to communicate effectively, have initiative, dependability and good judgment. Must be able to work collaboratively as part of an interdisciplinary group, as well as independently, and problem solve in a crisis situation. The employee shall work well under pressure meeting multiple and sometimes competing deadlines. The employee shall, at all times, demonstrate cooperative behavior with colleagues and supervisors. Demonstrates clinical competency as observed during supervisory visit.
EXPERIENCE AND EDUCATION:
  • Current California Registered Nurse License
  • One year of experience working as a professional nurse with background in caring for the terminally ill.
  • Must have a current California Driver's License and a car in good working order.
  • Must remain current in standards of practice for hospice care and pharmacology by taking agency and continuing education classes.
  • Hospice Nurse Certification is desirable
  • CPR Certification and IV skills required.
