Position Summary
Lakeview Health Center & Assisted Living is seeking a dedicated Registered Nurse (RN) to provide and supervise high-quality resident care in a long-term care environment. This role includes clinical assessments, care planning, medication administration, documentation, and supervision of LPNs, Nurse Technicians, and Nursing Assistants.
Essential Responsibilities
  • Provide direct nursing care in accordance with the Nurse Practice Act
  • Complete assessments and update individualized care plans
  • Implement medical orders and collaborate with physicians and interdisciplinary teams
  • Supervise and support nursing staff; assist with training and orientation
  • Ensure accurate, timely documentation
  • Maintain resident rights, safety, and confidentiality
  • Participate in required in-service education
  • Support person-centered care practices

MDS Responsibilities (Hillview Campus, if assigned)
  • Assist with accurate and timely MDS completion
  • Support restorative nursing programs
  • Apply clinical assessment skills to ensure accurate coding and compliance

Qualifications
  • Current Registered Nurse license in the State of Wisconsin
  • Graduation from an accredited nursing program
  • CPR certification
  • Strong leadership, communication, and assessment skills
  • Ability to meet physical demands, including lifting up to 60 lbs
  • Long-term care experience preferred
  • Must pass background check per Wisconsin Caregiver Law
