We seek a highly skilled and compassionate Registered Nurse (RN) to join our team at a Skilled Nursing Facility. This position is part-time/weekend for PM shift (3PM-11PM). As an RN, you will be responsible for ensuring that residents' needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures, and federal, state, and local guidelines. You will observe, evaluate, gather data, confer with, and be directed by the Director of Nursing to meet the patient's emotional, physical, and cognitive needs.

Responsibilities:

· Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition.

· Develop and implement individualized care plans based on residents' needs and goals.

· Coordinate care with other healthcare team members, including physicians, therapists, and other healthcare professionals.

· Ensure that all documentation and charting are accurate, complete, and current.

· Assist with activities of daily living (ADLs) as needed.

· Educate residents and families about their healthcare needs and treatments.

· Respond to emergencies and provide first aid as needed.

· Adhere to all policies, procedures, and regulations governing nursing care in the facility.

Qualifications:

· Valid and current RN license in the state of California

· Graduate of an accredited nursing program

· Minimum of 1-year experience in a skilled nursing facility, acute care setting, in-patient rehabilitation, or in-patient behavioral health setting

· Strong clinical skills, including assessment, planning, and implementation of nursing care.

· Excellent communication and interpersonal skills
