What You'll Do
  • Provide skilled pediatric nursing 1:1 care in patients' homes
  • Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies
  • Support hospital-to-home transitions for high-acuity patients
  • Educate and support parents, families, and caregivers
  • Assist with discharge support and care coordination
  • Train and support permanent field nurses during transitions
  • Provide first-shift and interim coverage as needed
  • Collaborate with Clinical Care Managers and interdisciplinary teams
  • Respond appropriately in emergency situations and changes in patient condition

Qualifications
Required:
  • Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
  • Current CPR certification
  • Ability to lift and assist pediatric patients as needed
  • Experience with Trach care and Ventilator management
  • High-acuity pediatric patients (preferred)
  • Emergency response management
Preferred:
  • Pediatric home health experience
  • High-acuity pediatric patients

    Schedule & Travel
    • Full-time position, with guaranteed hours
    • Flexible scheduling based on patient needs and Nurse availability
    • 5 working days per week, minimum 1 weekend day
    • On call and Staffing coverage responsibilities as needed
    • Local travel within approximately 30-40 miles
