Position Overview

The Weekend Admissions Registered Nurse is responsible for completing home health admissions, conducting comprehensive patient assessments, initiating plans of care, and coordinating services to ensure patients receive high-quality care from the start of their home health journey. This position serves as the primary admissions nurse Friday through Monday.

Key Responsibilities

โ€ข Complete home health admissions and Start of Care (SOC) assessments in accordance with physician orders and agency policies
โ€ข Obtain and document comprehensive medical histories
โ€ข Perform physical assessments, including vital signs, mobility evaluations, and safety assessments
โ€ข Complete OASIS documentation accurately and within required timeframes
โ€ข Develop individualized plans of care based on patient needs and physician orders
โ€ข Educate patients and caregivers regarding diagnoses, medications, treatment plans, and home health services
โ€ข Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members
โ€ข Ensure compliance with all federal, state, and agency regulations, including HIPAA requirements
โ€ข Identify and communicate changes in patient condition to the appropriate healthcare providers
โ€ข Facilitate referrals for additional home health services as needed
โ€ข Maintain timely and accurate clinical documentation

Qualifications

โ€ข Current and active Registered Nurse (RN) license in the State of Ohio
โ€ข Minimum of one year of nursing experience required; home health experience preferred
โ€ข OASIS documentation experience strongly preferred
โ€ข Experience with WellSky (Kinnser) preferred
โ€ข Strong clinical assessment and critical thinking skills
โ€ข Excellent communication and patient education abilities
โ€ข Current CPR certification
โ€ข Valid driver's license, reliable transportation, and automobile insurance
โ€ข Current TB test and physical examination in accordance with agency requirements
โ€ข Ability to attend orientation and training at the Aspire Home Healthcare of Ohio office
