Position Overview Collage Home & Community is seeking an experienced Registered Nurse (RN) to serve as an Alternate Nurse Supervisor supporting our Texas Licensed Home Health Services (LHHS) operations. This role is designed to initially begin as PRN and is expected to ramp quickly to a full-time position as new clients are onboarded and patient census grows. The Alternate Nurse Supervisor plays a critical role in ensuring continuity of RN supervision, clinical quality, and regulatory compliance. This position provides backup coverage to the Nurse Supervisor and supports day-to-day clinical operations in a growing, compliance-driven home and community-based care program.

Role Structure (Important) Initial Status: PRN (flexible scheduling) Expected Transition: Full-time as census and service volume increase Ramp Trigger: Growth in skilled nursing and CNA-supervised cases Coverage Role: Backup and support to the Nurse Supervisor to ensure uninterrupted RN oversight This role is ideal for an RN seeking flexibility initially with a clear path into a long-term leadership position.

Key Responsibilities Clinical Supervision & Coverage Serve as the Alternate Nurse Supervisor, ensuring RN supervision continuity when the Nurse Supervisor is unavailable Provide clinical supervision of RNs, LVNs, and Certified Nurse Assistants (CNAs) as assigned

Ensure CNA services are appropriately assigned, delegated, and supervised by nursing under Texas LHHS requirements Assessment & Care Planning Conduct or oversee patient assessments as delegated Assist with the development, review, and revision of physician-ordered plans of care Monitor patient progress and identify changes in condition requiring skilled intervention Compliance & Quality Assurance Support compliance with Texas Health & Safety Code Chapter 142 and 26 TAC Chapter 558 Maintain accurate, timely, and complete clinical documentation Participate in audits, surveys, and quality improvement activities Report incidents, adverse events, and clinical concerns according to agency policy Collaboration & Growth Support Partner with intake, operations, and leadership to support safe onboarding of new clients Assist in scaling clinical workflows as census grows Serve as a clinical resource to field staff and leadership

Qualifications Current, unrestricted Registered Nurse (RN) license in the State of Texas Minimum of 2 years of professional nursing experience Home health, community-based, rehab, or complex care experience preferred Strong clinical assessment, documentation, and communication skills Familiarity with Texas home health regulations preferred Current CPR certification Valid Texas driverโ€™s license and reliable transportation
