The Registered Nurse (RN) utilizes the nursing process to provide safe, effective, and patient-centered care while maintaining professional accountability for assigned patients. The RN evaluates the effectiveness of care provided by other direct caregivers and coordinates patient care collaboratively with interdisciplinary teams. This position upholds patient rights, maintains confidentiality, and performs all duties in alignment with Mercy’s mission, values, and Service Standards.

Position Details:

FSM Home Health provides short-term, insurance-approved care focused on delivering high-quality patient outcomes in the comfort of patients’ homes. Services are generally provided for 60 days or less, with care authorized in 60-day increments.

This role serves patients within a 50-mile radius of Fort Smith, Arkansas, supporting a defined service area limited to the state of Arkansas. RNs work a standard Monday–Friday schedule (8:00 AM – 4:30 PM) with occasional participation in weekend call rotation on a voluntary basis.

Home Health nursing offers a high level of autonomy, requiring strong time management, independent decision-making, and effective patient scheduling. The role involves detailed electronic documentation and consistent use of technology, making comfort with digital systems essential.

Candidates should be organized, adaptable, and confident working independently in the field. Reliable transportation is required, with mileage reimbursed per organizational policy.

Key Responsibilities

  • Deliver professional nursing care in the home setting following physician orders
  • Develop and update individualized care plans with measurable goals
  • Monitor patient progress and communicate changes to physicians and care team
  • Educate patients and caregivers on care plans, treatments, and medications
  • Coordinate services with interdisciplinary team members (PT, OT, ST, MSW, etc.)
  • Perform treatments and procedures in compliance with nursing standards and regulations
  • Assess home safety and report concerns, including potential neglect or abuse
  • Complete accurate, timely documentation (OASIS/Medicare/Medicaid guidelines)
  • Participate in on-call rotation, weekends, and holidays as needed
  • Support team collaboration, quality improvement, and patient satisfaction initiatives

Qualifications

  • Graduate of an accredited nursing program
  • Current RN licensure
  • Valid driver’s license
  • CPR certification

Preferred:

  • 1+ year of relevant nursing experience (home health or related field)
