Job Title: Registered Nurse, Clinical
Department: Clinic

Job Summary:
The Registered Nurse (RN) plays a critical role in supporting the Care Team POD model and managing nurse schedules to ensure efficient clinical operations. This role prioritizes patient-centered care, interdisciplinary collaboration, and proactive coordination to maintain continuity of care and optimize POD effectiveness.
Duties/Responsibilities:
Nurse Schedule & Clinical Coordination
  • Maintains nurse schedules to ensure adequate coverage across the clinic.
  • Administers treatments, injections, and immunizations.
  • Coordinates care for patients by working with the interdisciplinary healthcare team.
  • Adjusts staffing in real time based on patient volume and acuity.
  • Manages open triages and ensures timely resolution.
  • Assesses reasons for patient visits and directs ("jockeys") patients to the appropriate provider.
  • Prepares and pends orders for provider review.
  • Processes prescription requests.
  • Provides patient education on self-injection, new medications, treatments, or other care needs.
Care Team POD Model Responsibilities
  • Collaborates with providers, nurses, supportive services, and patient service representatives to maintain POD efficiency.
  • Ensures continuity of care within the POD by monitoring patient needs and follow-ups.
  • Handles open in-basket messages and coordinates appropriate interventions.
  • Facilitates effective communication with other Care Team RNs to support seamless care transitions.
  • Assists providers in delivering patient care.
  • Explains procedures and treatments to patients to gain cooperation, understanding, and reduce apprehension.
  • Aids providers during examinations and treatments.
  • Observes patients, records significant reactions/conditions, and notifies providers of reactions to drugs or treatments.
  • Assists providers with written prescriptions, refills, pharmacy phone-ins, and medication list documentation.
  • Performs duties of a Medical Office Assistant and/or Licensed Practical Nurse including taking vitals, updating patient information, collecting samples, and performing tests such as EKGs.
  • Participates in daily huddles and contributes to care coordination and quality improvement efforts.
Required Skills/Abilities:
  • Current RN license in New York State.
  • Minimum of 2 years of experience in outpatient or team-based care preferred.
  • Strong organizational, multitasking, and communication skills.
  • Proficiency in EMR systems (Epic experience a plus).
  • Commitment to patient-centered care and continuous improvement.
  • Ability to work collaboratively in a fast-paced environment.
Education and Experience:
  • Current New York State Registered Nurse license required.
  • Outpatient experience or team-based care experience preferred (2+ years).

Physical Requirements:
