Responsibilities• Provide skilled nursing care in the home setting• Follow individualized care plans and physician orders• Administer medications and treatments• Complete accurate documentation and patient assessments• Communicate changes in condition to the Clinical Supervisor• Assist with patient mobility, positioning, and transfers as needed

Qualifications• Active Arizona RN or LPN license or Compact License• Current BLS Certification (online-only courses not accepted)• Valid Arizona Driver’s License• Current TB test or chest X-ray within the past 12 months• Level One Fingerprint Clearance Card• Article 9 Training (assistance available)• Strong communication and critical thinking skills
