Registered Nurse (RN) - Home Health
Weekends | Day Shift | One-on-One Care
Location: New Stanton, PA
Schedule: Weekends | Hours Vary

Community Care, Inc. is looking for a compassionate and dedicated RN to provide home health care to our clients in the Westmoreland and Fayette County area. This is a great opportunity to build meaningful connections, deliver personalized care, and see the impact of your work every single day-all in a supportive home setting.
What You'll Do:
  • Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments)
  • Develop and implement individualized care plans
  • Educate patients and caregivers on managing conditions and medications
  • Collaborate with a multidisciplinary team to ensure continuity of care
  • Document visits and patient progress accurately and timely
• What You'll Need:
  • Current Registered Nurse (RN) license in the state of Pennsylvania.
  • Graduate of an accredited nursing program
  • Minimum 1 year of clinical nursing experience (Home Health experience preferred)
  • CPR certification (required)
  • Reliable transportation and a valid driver's license
  • A caring heart, strong independence, and a passion for pediatric care
