• Conduct thorough assessments to identify clients' unique social, emotional, and environmental challenges, utilizing standardized assessment tools and personal interviews.
• Develop and implement individualized service plans, which outline clear goals, actionable interventions, and mechanisms for progress tracking, ensuring a structured approach to client care.
• Provide individual and family counseling services tailored to client needs, guiding them through therapeutic techniques that enhance coping skills and resilience.
• Engage in crisis interventions, conducting risk assessments and designing safety plans that prioritize client protection and promote recovery stability.
• Collaborate with community agencies and organizations to ensure that clients have access to a comprehensive network of support services, advocating on their behalf to remove barriers to care.
• Participate in community outreach initiatives to raise awareness about service availability and educate the public on social issues such as substance abuse and domestic violence.
• Attend case conferences and team meetings, contributing insights and strategies for improving client care and program effectiveness.
• Document all interactions and progress in compliance with agency protocols, maintaining confidentiality and ensuring accurate records for ongoing review and assessment.

In this role, you will work primarily with individuals struggling with substance use disorders, often facing additional challenges such as mental health issues, homelessness, and domestic violence. Your approach will focus on empowerment, helping clients recognize their strengths and capabilities while providing the necessary resources and support to facilitate recovery and reintegration into society.

• Master's degree in Social Work from an accredited institution is required.
• A current and active state licensure as a Licensed Master Social Worker (LMSW) is essential.
• A strong understanding of social dynamics, trauma-informed care principles, and practices related to substance use.
• Excellent verbal and written communication skills, with the ability to demonstrate cultural competence in interactions with diverse populations.

• A minimum of two years of experience in a social service environment or mental health setting, particularly in roles centered around substance abuse treatment and recovery.
• Proficiency in using electronic health record systems and case management software.
• Experience with community outreach and awareness programs that address social issues affecting marginalized groups.
