At Arrowhead Family and Marriage Consulting LLC, located in Park Ridge, Illinois, we are proud to operate a multifaceted community health clinic dedicated to improving the well-being of families and individuals. In addition to providing obstetric, pediatric, general medical, family planning, and dental services, our committed team is equipped to address the complex social issues that impact our community. We prioritize patient confidentiality and accept Medicaid, Medicare, and most private health insurance plans to ensure access to comprehensive mental health care for all.

Work Environment

This remote position allows for flexibility while maintaining a collaborative approach with team members. You will provide essential counseling services through telehealth platforms, effectively reaching patients across various demographics. Your role will involve direct engagement with patients, healthcare providers, and other members of our multidisciplinary team, all while upholding Arrowhead's standards of care.

Key Responsibilities
  • Conduct thorough patient intakes that include diagnostic and functional evaluations, ultimately advising the primary care provider on treatment goals and plans.
  • Deliver brief, focused interventions for patients seeking mental health support, ensuring timely and effective care that meets established productivity targets.
  • Assist primary care providers in recognizing, treating, and managing mental health and psychosocial issues, while actively participating as a key member of the primary care team.
  • Monitor patient progress and coordinate behavioral health care services, linking patients with additional treatment providers as needed and with their consent.
  • Document patient assessments, treatment plans, and progress notes accurately in the electronic health record (EHR) to maintain compliance and support effective communication with other healthcare professionals.
  • Develop relapse prevention plans for patients and help them cultivate stability while also implementing strategies to assist patients in adhering to medical treatments.
  • Provide educational resources and self-management skills to empower patients in their own treatment journey, fostering independence and recovery.
  • Participate in consultations with clinic management and team members to evaluate the effectiveness of services and implement improvements based on patient outcomes.
  • Manage ethical responsibilities and confidentiality in accordance with the Code of Ethical Conduct for Clinical Professionals.
  • Perform additional duties as assigned to support team goals.
Patient Population / Client Focus

The Licensed Clinical Professional Counselor will serve a diverse clientele within the community, addressing a broad spectrum of mental health issues. This includes individuals with anxiety, depression, trauma, and other behavioral health concerns. Our clinic values a holistic approach to care, integrating emotional support with physical health services. As such, your work will directly contribute to improving overall patient outcomes and fostering a healthier community.

Qualifications
  • A minimum of a Master’s Degree in Clinical Counseling or a related field.
  • Current and valid Licensed Clinical Professional Counselor (LCPC) License to practice.
  • Robust understanding of mental health assessment, treatment planning, developmental psychopathology, and psychotherapeutic methods.
  • Ability to assess the unique needs of patients and to develop functional treatment plans that promote recovery and growth.
  • Excellent communication skills for effective engagement with patients, families, and interdisciplinary team members.
  • Knowledge of community resources and the ability to connect patients with necessary external support systems.
  • Proficient record-keeping and documentation skills to ensure compliance and continuity of care.
Preferred Experience
  • Bilingual proficiency in English and Spanish to better serve our diverse community is preferred but not required.
  • Experience in a team-based healthcare environment, particularly within an integrated care model.
  • Familiarity with electronic health records and telehealth platforms.
