This is a full-time position that offers remote work flexibility, allowing for work-life balance while supporting the emotional and mental well-being of patients in their homes or hospice facilities. Shifts will be scheduled based on departmental needs, including potential day, evening, and weekend hours to provide comprehensive support. You may be required to be on-call as part of our commitment to accessible patient care, demonstrating our dedication to being present for our patients and their families when they need us most.

• Conduct thorough psychosocial assessments to identify emotional, social, and financial needs of patients and their families.
• Provide individualized and group therapy sessions, focusing on adjustment disorders, grief counseling, and coping strategies to help patients navigate their healthcare experience.
• Develop and implement tailored discharge plans in collaboration with multidisciplinary teams, ensuring smooth transitions to post-discharge services.
• Coordinate with community organizations and resources to facilitate ongoing support and continuity of care after discharge.
• Engage in crisis intervention, offering immediate support during critical moments to patients and their families.
• Maintain meticulous documentation in compliance with hospital policies while ensuring legal and ethical standards are met.
• Participate in committees focused on improving social work practices and enhancing the quality of care provided.

You will be working closely with patients at various stages of life who require hospice care, often dealing with complex emotional and social situations related to terminal illness. This role requires a nuanced understanding of the unique challenges faced by individuals and their families in these circumstances, emphasizing dignity and compassionate support.

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• Thorough understanding of mental health disorders, medical terminology, and effective crisis intervention techniques.
• Strong verbal and written communication skills paired with exceptional interpersonal abilities.

• Two years of direct experience in a healthcare or hospital setting, especially in hospice care, is preferred.
• Experience with interdisciplinary collaboration and participating in multidisciplinary team meetings to discuss patient care strategies.
• Previous work involving end-of-life care planning and emotional support interventions.

The salary for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and expertise.
