You will collaborate closely with our hospice center team, engaging in virtual meetings, consultations, and collaborative care approaches. Regular business hours are expected, with some occasional evening and weekend availability based on client needs and program activities.

• Conduct thorough assessments of clients’ social, emotional, and environmental challenges, utilizing tools such as interviews and standardized assessments.
• Formulate personalized service plans that identify specific goals and interventions, and provide regular progress updates.
• Offer supportive counseling to individuals and families coping with life-altering diagnoses, bereavement, or other challenges inherent in hospice care.
• Coordinate with clients and their families to outline and access community resources, which can include financial assistance and emotional support groups.
• Implement crisis intervention strategies, including risk assessments and safety planning, to ensure immediate client needs are met.
• Collaborate with external agencies, healthcare providers, and community organizations to secure holistic care for clients and advocate for their needs.
• Engage in community outreach initiatives aimed at raising awareness about hospice services and resources available to the public.
• Participate actively in case conferences, team meetings, and ongoing professional development sessions to enhance team collaboration and efficacy.

Your work will primarily focus on individuals and families in hospice care, providing them with not only the emotional support they need but also practical assistance in navigating the complexities of end-of-life planning and care. You will become a trusted advocate and resource as clients face significant life transitions and health challenges.

• A Master’s degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Nevada is mandatory.
• Knowledge of trauma-informed care principles, with a focus on enhancing client resilience and well-being.
• Strong communication skills, cultural competency, and an ability to engage with diverse populations effectively.

• Prior experience in a social service agency or healthcare setting, particularly in hospice or end-of-life care.
• Background in crisis intervention, grief counseling, or case management specific to healthcare settings.

The compensation for this position ranges from USD 60,000 to 100,000 annually, commensurate with experience and qualifications. Additional benefits include:
