Work Environment

This role is entirely remote, offering you the flexibility to work from the comfort of your home while still being able to positively impact the lives of our clients at our affiliated Hospice Center located in Elgin, Illinois. The position requires a full-time commitment of Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends as needed to address client availability and requirements.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans tailored to meet the unique needs of each client, focusing on their mental health challenges and personal goals.
  • Facilitate individual and group therapy sessions, employing evidence-based therapeutic techniques to foster coping strategies and resilience in clients facing mental health crises.
  • Collaborate effectively with a multidisciplinary team, ensuring the seamless integration of care by sharing insights and strategies aimed at achieving common objectives focused on patient empowerment and recovery.
  • Maintain meticulous records of client progress and treatment outcomes, adhering to ethical guidelines and organizational policies while using the electronic health records (EHR) system effectively.
  • Actively engage with clients from diverse backgrounds, respecting their unique beliefs and traditions while fostering a supportive therapeutic environment.
Patient Population / Client Focus

This role focuses on clients who are experiencing major mental illnesses and severe emotional disorders, many of whom are facing complex life transitions, particularly those in hospice care. You will play a crucial role in offering emotional support, practical coping strategies, and facilitating access to necessary resources, thus enhancing their overall quality of life during challenging times.

Qualifications
  • Must hold a current and valid LCSW license.
  • Bachelor's degree in social work or a related field.
  • Demonstrated expertise in assessment, diagnosis, and treatment methodologies for mental health issues.
  • Excellent verbal and written communication skills, essential for engaging effectively with clients, families, and multidisciplinary team members.
  • Strong organizational skills that enable the planning and execution of treatment strategies in a fast-paced environment.
Preferred Experience
  • A minimum of two years of clinical experience is preferred, ideally within a hospice or palliative care setting.
  • Experience with crisis intervention and risk assessment for clients undergoing significant emotional distress.
  • Familiarity with electronic health record (EHR) systems to ensure accurate documentation and adherence to best practices in patient care.
Compensation and Benefits
  • The compensation for this role ranges from $85,000 to $90,000 annually, commensurate with experience.
  • We offer a comprehensive benefits package that includes health, dental, and vision insurance, alongside a retirement savings plan with employer-matching contributions.
  • Employees are entitled to generous paid time off (PTO) and sick leave, promoting a healthy work-life balance.
  • Life insurance, as well as robust continuing education opportunities, are provided to encourage ongoing professional development.
  • As we value our dedicated team members, a sign-on bonus is currently being offered for new hires.
