You will engage with clients primarily through virtual meetings, phone calls, and telehealth platforms, ensuring you can provide support during regular business hours as well as occasionally during evenings and weekends when client needs arise. • Conduct comprehensive psychosocial assessments to identify clients’ social, emotional, and environmental challenges during end-of-life care.
• Develop personalized service plans that incorporate measurable goals, appropriate interventions, and continuous progress assessments to ensure effectiveness.
• Facilitate counseling sessions with individuals and families coping with grief, loss, and other life transitions, providing empathetic support and guidance.
• Collaborate closely with clients to engage them in developing tailored care plans and connect them with necessary community resources for additional support.
• Provide crisis intervention and risk assessment to clients in distress, implementing safety planning as necessary to ensure their wellbeing.
• Work in cohesion with various community agencies and organizations to coordinate services and champion clients’ rights and needs.
• Engage in community outreach initiatives to promote agency services and raise awareness about hospice care, enhancing community knowledge and access to resources.
• Participate actively in case conferences, team meetings, and ongoing training sessions to foster a collaborative work environment and enhance personal professional development.

You will primarily work with clients in hospice care, focusing on individuals and families dealing with terminal illnesses and complex emotional challenges. Your role will encompass understanding the unique needs of these clients, providing guidance, and helping them navigate their circumstances while respecting their dignity and choices.

• Master's degree in Social Work from an accredited institution.
• Valid state licensure as a Licensed Master Social Worker (LMSW).
• In-depth knowledge of social service systems, community resources, and trauma-informed care principles.
• Strong communication skills, including the ability to engage with diverse populations and demonstrate cultural competence.
• Critical thinking and problem-solving abilities to effectively address client needs and challenges.

• A minimum of two years of experience in a social service agency or hospital setting, particularly in palliative or hospice care.
• Familiarity with case management practices and methodologies specific to social work in healthcare settings.
• Experience in conducting assessments and developing service plans tailored to individualized care.
