Position Overview

Brighter Life Social Work is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in a remote position, primarily serving clients in a hospice care setting. Work Environment

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify clientsโ€™ social, emotional, and environmental challenges during end-of-life care.
  • Develop personalized service plans that incorporate measurable goals, appropriate interventions, and continuous progress assessments to ensure effectiveness.
  • Facilitate counseling sessions with individuals and families coping with grief, loss, and other life transitions, providing empathetic support and guidance.
  • Collaborate closely with clients to engage them in developing tailored care plans and connect them with necessary community resources for additional support.
  • Provide crisis intervention and risk assessment to clients in distress, implementing safety planning as necessary to ensure their wellbeing.
  • Work in cohesion with various community agencies and organizations to coordinate services and champion clientsโ€™ rights and needs.
  • Engage in community outreach initiatives to promote agency services and raise awareness about hospice care, enhancing community knowledge and access to resources.
  • Participate actively in case conferences, team meetings, and ongoing training sessions to foster a collaborative work environment and enhance personal professional development.
Patient Population / Client Focus

You will primarily work with clients in hospice care, focusing on individuals and families dealing with terminal illnesses and complex emotional challenges. Qualifications
  • Master's degree in Social Work from an accredited institution.
